Nurse Practitioner Hourly
PRIDE Health is seeking Nurse Practitioners for our well-known national client!The Nurse Practitioner will provide care to patients in a long-term care and personal care setting visiting nursing homes, assisted living facilities, and homes.
These are all contract opportunities for about 12
weeks.
(Milage to and from appointments is expensed)Monday through Friday schedule, 40-hour work weeks, 8AM-5PMPay rate range (based on experience) $65-$70 HourlyNurse Practitioner will be traveling to primarily nursing homes in the Ambler, PA area such as Elkins/Silverstream, Elkins Park, and Spring House, PA Specific Duties:
1.
Conduct comprehensive assessments.
2.
Manage patients through an acute medical situation or an exacerbation of a chronic condition including testing, diagnostics, and interventions.
3.
Ensure accurate and complete documentation, including ICD10 coding.
4.
Communicate and collaborate with the interdisciplinary care team and primary care physician.
5.
Conduct advanced illness and advanced care planning conversations.
Qualifications:
Active Pennsylvania NP LicenseBLS CertificationActive ANCC or AANP Certification2
